Punctures happen from many scenarios. The most common is when foreign objects fall, they will puncture the system. Heavy foot traffic leads to deterioration and even loose flashings will cause holes in your roof. 

Cracks can be caused from building movement, and/or insulation movement. Fractures can potentially be from the constant cold and hot temperatures and aging of your roof.
